The U.S.S. Enterprise is sent to deliver a new medicine to Elba II, a planet with a poisonous atmosphere that houses an underground facility for the incurable, criminally insane; it is hoped that the revolutionary drug will eliminate mental illness forever. But when Captain Kirk and Lieutenant Commander Spock beam down to the facility they find that Governor Donald Cory is behaving very oddly. However, this is not the real Governor - the asylum has been taken over by the insane Garth of Isar, once a celebrated Federation starship captain, who has learned the technique of cellular metamorphosis. Garth attempts to convince the two Starfleet officers that he is the head of the facility so that he can gain control of the Enterprise, which he plans to use to conquer the galaxy. However, when Kirk and Spock find the real Cory in Garth's cell, Garth takes them prisoner, and takes on Kirk's form. He contacts the Enterprise and instructs Chief Engineer Scott to beam him aboard; but Scotty is suspicious when his captain fails to give the correct password. Thwarted, Garth and his fellow inmates resort to torturing first Cory, and then Kirk himself, in an attempt to get the captain to reveal the password. Can Kirk and Spock outwit the self-proclaimed 'Master of the Universe' and escape from the asylum?
