Captain Kirk is shocked to learn that Doctor McCoy has contracted a fatal disease known as xenopolycythemia, and has just one year to live. When the U.S.S. Enterprise then manages to evade an attack by chemically-fused missiles, the rockets are determined to originate from an asteroid named Yonada. However, closer analysis shows that the asteroid is actually a nuclear-powered spaceship, and that it is on a collision course with the Federation planet Daran V. Kirk, McCoy and Lieutenant Commander Spock beam down to investigate and discover that the interior of the asteroid is hollow, and is occupied by a humanoid race of people. The ancestors of these inhabitants built the vessel so that they could escape the destruction of their solar system, but the controls have become defective and Yonada is now out of control. The people are ruled by Natira, a priestess who takes her orders from a central computer designated the Oracle; the machine maintains peace by implanting the inhabitants with devices called 'Instruments of Obedience', which punish wrong thinking. While Kirk and Spock search for the controls to redirect the ship, McCoy and Natira fall in love. But when Kirk and Spock return to the Enterprise, McCoy decides to spend his remaining time with the woman he has come to love; he remains on the asteroid, marries Natira and accepts an Instrument of Obedience…
